Macau Day in Taipa Submission Confirmation

Your appointment is confirmed for Wednesday, October 23, 2019 at the Ocean Club, No. 1000 Avenida dos Jardins do Oceano. Please arrive at your appointed time with your completed documents.

PLEASE READ CAREFULLY THE INSTRUCTIONS BELOW. Our requirements for services in Macau are different from those in at the Consulate General in Hong Kong.

PAYMENT INFORMATION

Due to Department Regulations, we cannot accept credit card or cash payments. The only acceptable form of payment will be a bank draft/cashier’s check. Bank draft / cashier’s check should be made payable to “USDO Symbol 8768” and must be drawn in U.S. dollars on a bank in the United States and must have been issued within the last five months. We understand that in Macau, HSBC, BNU, Bank of East Asia (BEA), and Banco Tai Fung provided this service in the past, but please check with your preferred bank to see if they are able to assist you with your request. Please see the sample of the check (PDF 201 KB), which you may print out and take to the bank.

Notarized documents are US $50 per signature of the Consular Officer. The amount on the check must EXACTLY match the services you are applying for; if it is too much money or too little money, you will have to obtain a new check. If you are not sure how many notarials your document requires, please scan and send the document to us at acshk@state.gov on or before noon on Monday, October 21, 2019 with the subject line MACAU DAY: NOTARIAL SERVICE and we will inform you how many notarials your document requires and how much money you will need to prepare for your check.

DOCUMENTATION

Please bring with you a government-issued photo ID. A passport or Macau or Hong Kong ID is best. You must also bring the document you want notarized, complete with the appropriate names, places, and dates (but don’t sign it; you will sign in front of the Consular Officer).

DOCUMENTATION

In addition to bringing original passports, birth certificates, CRBAs, and/or other required documents, you will need to bring a photocopy of each original document (no need to copy the application form) with you. For passports, please copy only the biographical/photo page.

Please be reminded to email scanned copies of all required application forms (DS-11, DS-82, DS-5504) and original documents (passports, U.S. birth certificates, CRBAs) to acshk@state.gov on or before noon on Monday, October 21, 2019 with the subject line MACAU DAY: PASSPORT SERVICE.

If you require an additional service, such as a notarization, please arrive at the venue by 10:30 a.m. Notarized documents are US $50 per signature of the Consular Officer.

If you require an additional service, such as a notarization, please arrive at the venue by 10:30 a.m. Notarized documents are US $50 per signature of the Consular Officer.
